What schools can enroll in the program?
We have been working closely with the MIAA member schools and have opened up the opportunity for any school, both public and private, to enroll in our program.
How do I enroll my school in the MCMC program?
In order for your school or district to be fully enrolled, you must first complete the Enrollment Form on-line and then complete the Expectations Form. These forms can be found on our website at: www.massconcussion.org and click on the Forms tab.
How do I send in the completed forms?
The enrollment application is an on-line Google Form. It has required information to fill out and automatically saves the responses. The Expectations Form is an edit ready PDF and is best viewed in Adobe. After completing the information, you must save it to your computer before attaching it to an email. (If you do not have Adobe, please print the document, fill it out, scan it to your computer and attach it to an email.) The completed Expectations Form should be emailed to: firstname.lastname@example.org.
How do I receive ImPACT™ Concussion Testing for free?
This question depends on if your school or district is or is not an existing ImPACT™ customer.
A. If your school is not an existing customer, you will be set up within the ImPACT™ Customer Center Site. Your system administrators, such as the Athletic Director, Athletic Trainer or School Nurse, will receive an email from ImPACT™. This email will contain information about your user name and password and how to log into the site. Please free to contact MCMC for help and guidance with your concussion testing.
B. If your school is an existing customer, you will need to fill out the ImPACT™ Data Transfer Request Form. This form can be found on our website under the “enrollment forms” tab. The completed ImPACT™ Data Transfer Request Form should be emailed to: email@example.com.
How long does the transfer request take?
It will take ImPACT™ a day to make the transfer. You will not have any delay on your end. You can continue to use the customer center site as usual.
Will I need a new user name or password? Will my customer ID code change?
No. Nothing will change on your end. Your school will continue to use the same ID code as it had in the past.
Where can I go to find help with the ImPACT™ customer center site?
ImPACT™ has created a great “Quick Reference Guide”. Here you will find answers to all your questions in regards to ImPACT™ testing and use. You will find this guide at the bottom of the page of the customer center site.
What is the web address for the customer center site?

You will then enter your school’s customer ID code. Your school’s name will appear under “ImPACT Applications”. Click “launch baseline test”.
